Educational Event Application (PDF Format)
This form can be used in applying for any educational event offered during the season.

Program Effectiveness Evaluation Sheet Use this form to evaluate an education event or exam that you have recently taken through ASEA Central. Below are some different form options:

PDF Format - Print this form and fax or mail back to the PSIA/AASI Central Office.

MS Word Format - Print this form and fax or mail back to the ASEA Central Office.

MS Word Format (Tab Fill in) - When you select this form, save it onto your computer. You can fill in the fields by tabbing through the differnet fields and email this form back to the ASEA Central office at education@psia-c.org.

ASEA-C Merchandise Order Form (PDF Format)
Order any currently stocked manuals, videotapes and accessories with this form. (Please note that items that are out of stock will have to be ordered from the National website.)

Waiver Request Form (PDF Format)
This form is for active members who cannot fulfill their education requirements for the current season.

Outside Division Education Verification Form (PDF Format)
Take this form with you when you want to receive education credit for an education clinic in another division.

Non-ASEAC Credit Request (PDF Format)
This form is to request credit for a non-ASEA event. Your request for education credit must be sent to the ASEA-C office at least two weeks before the event to receive approval.

Use this form to request a Traveling Examiners Clinic for your snowsport school. Please send your requests in advance to ensure staffing.

Unscheduled Event Application (PDF Format)
Use this form to request a Unscheduled Event at your area. Please send your requests in advance to ensure staffing.

Snowsport School Directors Exam Shadow Program (PDF Format)
The purpose of this program is to help directors train their instructors for the certification process. Please submit application in advance for approval.
